What is Midwifery?

Midwifery is a medical profession, independent from obstetrics, which deals with the physical and emotional needs of women during pregnancy, childbirth, and the postpartum period. It has been practiced for centuries in many cultures all over the world. In the United States, midwives are licensed by individual states. There are also many unlicensed midwives who practice for themselves and others in the community.

Like all medical professionals, a midwife is bound by law to uphold certain standards of ethics and practice according to specific guidelines. A midwife must have an appropriate level of training and education to practice safely and effectively. Midwifery is a primary health care provider (PHC) practice, delivering babies in place of medical personnel or physicians.

Midwives do not deliver babies; they simply provide health care to pregnant women. In fact, midwives are not legally allowed to perform any procedure that may result in a live birth (such as cutting an umbilical cord or tying it). Midwives do not take care of infants once the birth is over, but rather refer women to specialists. Midwives can actively assist in the delivery process (induce, monitor fetal heart rate or other signs), monitor the mother for signs of distress during labor, and help to deliver the baby.

There are various categories of midwives;

Categories of midwives  include;

  1. Traditional midwives
  2. Legal midwives
  3.  Lay midwives
  4. Certified professional midwives,
  5. Certified nurse-midwives (CNMs).
  • Traditional Midwife refers to a practitioner who has a degree in midwifery from a recognized institution of higher learning and applies this knowledge in an actual clinical setting. In other words, they attend births as their primary career but they might have additional nursing or medical training such as BLS/CPR. Traditional midwives are not required to be certified or licensed. They are the only midwives in the United States that can practice on their own. Traditional midwives have been practicing for over 10,000 years in many cultures and countries, including China, Japan, India, Korea and Egypt.
  • Legal Midwife: – A legally trained member of the American College of Nurse-Midwives who is certified by the state board of nursing that oversees all home births.
  • Lay Midwife refers to a midwife who is not licensed or certified by a state board of midwifery but has accepted the responsibility for providing safe home birth care in their community. Lay midwives are self-employed, practice independently and do not charge for their services. They typically share their knowledge with others in the community who want to learn about childbirth.
  • Certified Professional Midwife: – A midwife who has completed a midwifery education program accredited by the Board of Nurse-Midwives of America (BNMA), completed a Bachelor’s Degree with a major emphasis on human sexuality, and meets certain professional criteria for board certification. The BNMA is the national organization for nurse-midwives with membership in over 40 countries. The graduates of this profession are able to practice in many different settings including ones that do not have formal medical training.

Importance of Midwifery;

  1. Midwives help lower rates of unnecessary newborn mortality by providing women with the health care tools they need to effectively manage their pregnancies.
  2. Midwives contribute by keeping up-to-date on the latest research on women’s health related issues, including prenatal care, childbirth, postpartum recovery and infant feeding.
  3. Midwives, who care for women during the all-important first days and weeks of motherhood, are uniquely qualified to recognize any health problems that might develop later on.
  4. The presence of a midwife also allows the mother to spend more time with her newborn baby as she recovers from childbirth and is better able to manage any complications that arise in the course of her recovery.
  5. Midwives are trained professionals who work hard to make sure that childbirth is a positive experience for both mothers and babies.
  6. Midwives help improve the overall quality of life for women and their families by providing both health care and emotional support.
  7. Midwives play an important role in the overall well-being of our community.
  8. Midwives help make sure that women who need health care services are able to obtain these services early enough in their pregnancy so they do not have to resort to abortion.
  9. Midwives help provide an environment where childbirth can occur naturally and safely, and births can be welcomed by family and friends as a joyous event.
  10. Midwives help make sure that women who want to breast-feed are able to do so successfully.
